Physical Activity and Dementia Risk: A Complex Relationship
A large systematic review and meta-analysis suggests that leisure-time exercise is associated with lower dementia risk, while occupational physical activity may increase the risk.

Physical activity has long been touted as a key factor in maintaining brain health and reducing the risk of dementia. However, a recent study has challenged this assumption, suggesting that the relationship between physical activity and dementia risk may be more complex than previously thought.
The study, which spanned 74 cohort and case-control studies from 2021 to 2026, included over 4.2 million people and found that high levels of physical activity were associated with a 20% lower risk of all-cause dementia over 10 years compared to the lowest level of physical activity. Leisure-time activities such as walking, running, swimming, cycling, or playing sports were also tied to lower dementia risk.
However, the study also found that occupational physical activity was associated with an increased risk of dementia. This may be due to the fact that physically demanding jobs often involve more stress and hazardous environmental exposures, which can increase the risk of dementia.
The findings of this study are significant because they challenge the long-held assumption that "every move counts" for brain health. Instead, the study suggests that the type of physical activity and the context in which it occurs may be more important than the amount of activity itself.
The researchers behind the study noted that leisure-time exercise may support cognitive health through several pathways, including social interaction and cognitively demanding tasks. However, they also acknowledged that meta-analyses of observational studies of leisure-time physical activity remain susceptible to reverse causation, which may have exaggerated the magnitude of the observed protective association between leisure-time physical activity and dementia.
